- Our plaza is in between Highway 400 and Jane Street.
- Our plaza is on the south side of Rutherford Road. Canadian Tire is on the north side of Rutherford Road.
- Our plaza is across from Vaughan Mills mall (directly across from the now-closed Toys R Us).
- In the same plaza as: Starbucks, Chatime, Yokai Izakaya and Marcello’s Pizzeria.
CHECK-IN
CANCELLATION
Please provide at least 48 business hours notice if you need to cancel your appointment to avoid a late cancellation fee.
(Ex: call by Thursday to cancel a Monday appointment; call by Wednesday to cancel a Friday appointment).
How to CANCEL an appointment:
If you have received a reminder email, please use the cancellation link in that email to cancel your appointment. Please note that reminder emails are outgoing only, so replies to them are not received.
OR
If you did not receive a reminder email, please call us to cancel. If after hours, please leave a voicemail message with the following information:
1. The appointment time & appointment date that you would like to cancel
2. Patient’s first & last name
3. Patient’s phone number
WAITING ROOM ETIQUETTE
To ensure a pleasant and respectful atmosphere in our waiting room, we kindly ask for your cooperation with the following etiquette guidelines:
Arrive On Time:
- Please arrive on time or at most 5 minutes early.
- Late arrival might not be accommodated.
- If you are very early for your appointment, please kindly wait in your car until 5 minutes before your appointment’s time.
Accompanying Guests:
Please attend the appointment alone unless patient is one of the following, then one family member can accompany:
- a minor (17 years old and under; both parents can attend, but please do not bring siblings)
- needs a translator
- needs mobility support
- has memory recall issues (such as developmental delays, dementia, etc.)
